WASHINGTON D.C. (WKZO) -- The National Transportation Safety Board investigation into a pair of Amtrak incidents say safety measures were forgotten during maintenance operations, resulting in a death in Illinois and injuries in Michigan.

The accident report on a car-train collision last year in Madison, Illinois last February says a woman was killed because repair crew left lights and crossing barriers switched off at a crossing.

A train incident that injured 14-people near Niles last October was blamed on a switch that was left switched in the wrong way, diverting a speeding Amtrak train onto a side track, where the “Wolverine” service stopped just short of crashing into train cars parked on a siding.

The report says that over a hundred passengers were put in harm’s way because of the maintenance crew’s errors.

Amtrak has 90 days to respond to the report. The NTSB included in their reports, specific recommendations to avoid similar incidents in the future.
