DETROIT (WKZO) -- General Motors is accused of overlooking a Maryland crash report in 2005 that indicated a potential problem with ignition switches on a Chevrolet Cobalt.

The "Detroit Free Press" reports a teenager was killed in that crash, with police noting the ignition was turned to "accessory" and not "run," which prevented the airbags from deploying. GM was told about the report in 2007, but didn’t issue a recall for the problem until earlier this year.

Congress, the National Highway Traffic Safety Administration and the Justice Department are all involved now in the investigation.
